New this year!  The North Dakota High School Activities Association (NDHSAA), now requires all student athletes to have an annual Sports Physical.  They have also created a new form that must be completed in full and signed by the athlete's parent/guardian prior to their exam.

Athletes are invited to visit Craven-Hagan Clinic on Tuesday, August 10th between the hours of 4 - 6 p.m. to receive their annual sports physical - no appointments needed.  The cost is only $15 cash or check, as no insurance forms will be filed.  Please bring a copy of current immunization record along when attending the sports physical on Tuesday, August 10th; no immunizations will be given on this day.  Athletes must present their completed form containing their parent/guardian's signature and cash payment to clinic staff when they arrive for their sports physical exam.   All proceeds will be donated to the area school of the athlete's choice.
