| Mercy Medical Center offers employees a variety of Benefits. A summary description of the most common benefits are listed below. Feel free to ask any questions you may have about this benefit package. These benefits may be subject to change. |

| Paid Annual Leave (PAL) is designed to include Vacation, Holiday and Sick Hours in a single category. You may use any number of hours in your PAL balance in any way you want and at any time, provided departmental scheduling allows, adequate notice is given, and approval is received from your supervisor. |

| As part of the PAL Program a certain number of hours will also accrue and be set aside in an extended account. These hours are generally used for an extended illness. |
 |

| Employees hired for a minimum of 32 scheduled hours per pay period begin accruing PAL/EXTENDED Sick Leave hours on their first day of employment, however, new employees must complete six (6) months of employment to be eligible for compensation from PAL or EXTENDED Sick Leave Hours. During the first year of employment, employees (with hire dates prior to July 1st) are eligible to use the number of PAL hours accrued for that calendar year. All part-time employees earn pro-rated benefits based on scheduled hours. Flexible status employees receive a 5% differential on their base hourly rate in lieu of benefits. |

| Employees accrue time off based upon the scheduled hours each pay period and the number of years of service with Mercy Medical Center. The yearly accruals in the following table are based upon 80 hours per pay period: |

| Eligible employees accrue extended hours from their first day of employment and accrue a maximum of 480 hours (60 days) in their extended sick leave account. Extended sick leave may be used as follows: |

| Mercy Medical Center will designate a specific day on which to recognize seven (7) of the eight (8) holidays yearly. The holidays will be: New Years, Good Friday, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas. The eighth (8th) is recognized as the employee's Birthday holiday, which may be used any time during the year. Employees who are scheduled off during a designated holiday will be paid for their scheduled shift. These hours will be subtracted from their PAL account. Employees working on designated holidays will receive normal pay and no deductions will be made from their PAL account for that holiday. |

| Pre-Tax Premium Conversion Plans for Health, Vision, and Dental.
Group Life - All full and part-time employees are covered by a Term Life Insurance Policy. The amount of Life Insurance is calculated on the employees annual earnings, based on scheduled hours. Mercy Medical Center pays the entire premium for this Group Life Insurance. Available options at employees expense include Voluntary Life, Spousal Life and Children Life. Long Term Disability - Mercy Medical Center provides this benefit to all full & part-time employees at no cost to the employee.
Medical - Full and part-time employees are eligible to participate in the Self Funded Health Insurance Program. Enrollment is possible the first full month of employment.
Dental/Vision - Insurance is available to the full-time and part-time employee, the premium is paid in full by the employee.

| Every new employee will be provided with a pre-employment health screening. Thereafter all employees will be required to have an Annual Mantoux/TB test. Employees will have the option to do a Blood Screening (32 panel blood draw) yearly. The cost will be paid by Mercy Medical Center. |

| Employees are encouraged to purchase a meal ticket to be used in the Cafeteria. Employees will receive a 25% discount on the price of their meal ticket. |

| Employees are eligible participants for the Pension Plan after completing one year of service and working at least 1000 hours a year. Employees will be 100% vested after working 1000 hours for 5 years. |
